Summary: The Bariatric Chart Reviewer is responsible to the Manager and responsible for chart review and other methods for the collection of data specific to Bariatric Services. Provides internal consulting to those departments involved in tracking, coordinating, and monitoring Bariatric data collection to measure outcomes. Reviews and collects data from medical records and other documents, as required, and compares them against predetermined quality and national indicators to provide staff support for the quality assurance functions.

Responsibilities:
Clinical Knowledge: Data collection and identification of areas for quality improvement requires clinical knowledge and understanding of patient care.
Identifies 100 percent of metabolic and bariatric surgery patients for inclusion in the MBSAQIP data registry. Collects preoperative, intraoperative, 30-day postoperative, and long-term follow up data components for the program through the effective utilization of the hospital medical record systems and through a coordinated effort with individual surgeon office staff.
Identifies streamlining and process improvement opportunities in the data collection process. Demonstrates appropriate utilization of resources necessary to obtain valid, reliable data for entry into the program.
Utilizes software and applications for data collection and analysis.Responsible for the accurate, complete and timely entry data into the program's database.
Requisitions necessary charts from the Medical Records department and performs the reviews. Obtains and reviews other documents as appropriate and necessary
Prepares summary reports of findings from reviews. Maintains files of all documentation of chart reviews and insures that all documents are held in safe keeping. Participates in data compilation for audits and (re) accreditation or site visits.
Assists in designing and developing educational material for special projects for staff and community.
Remains abreast of latest updates/changes in relevant registry programs and data base programs.
